About Supported Living
Our Supported Living Services
1. Step-Down Rehabilitation
Award-winning NHS partner service supporting safe hospital discharges, preventing delayed transfers, and reducing readmissions. Focused on recovery, stabilisation, and reintegration into the community.
3. Acquired Brain Injury (ABI)
Specialist neuro-informed programmes with therapy carry-over, daily living skills training, and behaviour regulation, helping people regain independence and quality of life.
5. Dual Diagnosis (Mental Health + Substance Misuse)
Compassionate, non-judgemental support for people with co-occurring needs. We provide clear, integrated pathways with joint-working protocols, ensuring there is “no wrong door” and no one is excluded because of complexity.
2. Learning Disability & Autism
Specialist, person-centred support aligned to Building the Right Support. We provide structured routines, communication aids, PBS strategies, and sensory planning; working closely with families to help individuals live with dignity and purpose in safe, low-arousal environments.
4. Crisis & Complex Mental Health
24/7 intensive support for people in crisis or at risk of admission. Delivered in partnership with NHS crisis teams and designed to stabilise, reduce distress, and support recovery.
6. 24-Hour Supported Living
Round-the-clock staffing, waking night and sleep-in services, robust governance, and CQC-registered personal care where required.
What We Offer
24/7 staffed Supported Living
(Waking night/sleep-in as clinically required), with outcomes-driven support plans and positive risk-taking. Supported living keeps tenancy rights separate from care and is regulated for Personal Care by the CQC.
Positive Behaviour Support (PBS)
For people whose behaviour challenges, reducing restrictive practices and improving quality of life.
Step-Down Services (Hospital Discharge & Flow)
Short-to-medium-term placements that enable safe, timely discharge, reduce length of stay, and support re-integration to community life. We work to CPA plans, s117 aftercare and PHBs.
Recovery, Skills & Independence: daily living skills
Medication support, budgeting, community access, education/employment pathways, digital inclusion and peer support delivered through trauma-informed practice.
Clinical & MDT Partnership
Close liaison with NHS clinicians and local authority teams; access to neuro/psychology, OT and SLT where needed for ABI and complex LD&A.
Housing with
Support
Quality homes near transport and services, tailored to individual needs (e.g., low-arousal environments for autism).
